I hereby authorize McGlynn & Company, hereinafter called COMPANY, to initiate debt entries to my account indicated at the depository financial institution named hereinafter called DEPOSITORY, and to debit the same to such account I acknowledge that the origination of ACH transactions to my account must comply with the provisions of US law. This authorization is to remain in full force and effect until COMPANY has received written notification from me of its termination in such time and in such manner as to afford COMPANY and DEPOSITORY a reasonable opportunity to act on it. McGlynn & Company requires a minimum of 15 days to process this form before your accounts will be debited. Please continue to pay by check until you receive notice from McGlynn & Company that the automatic debit has been set up. Debit authorizations MUST provide that the receiver may revoke the authorization only by notifying the originator in the manner specified in the authorization.
